Give It a Try – Girls’ Rugby Programme Summer 2020 Participant Registration form

Dear Parent/Guardian,

Please provide the following information in order to register your child for the Give It A Try (GIAT) Girls’ Rugby Summer Programme. The club collects the following information so that you may be contacted in case of emergency or with information relating to the club and the GIAT summer programme. Please note that in addition to this form, Parents/Guardians are required to complete the IRFU Return to Rugby Personal Declaration for all GIAT participants.

COVID-19 Precautions

Whilst we as a club will be taking comprehensive measures to ensure we adhere to all safety guidelines, we would ask that all players/parents/guardians/spectators follow the below guidelines;

• Don’t Share water bottles
• Wash hands before and After training
• Don’t spit or clear nasal passages on the pitch
• Adhere to social distancing measures
• Do not shake hands, fist bump or high five
• If you feel unwell, notify your coach immediately
